A delay in developmental age compared to CA of 50% or a score 2 SD below the mean in one of the developmental domains or sub-domains; OR a delay in developmental age compared to CA of 25% or score 1.5 SD below the mean in two or more of the developmental domains or subdomains

Vision Impairment
List of diagnosed conditions includes (but is not limited to):
- Retinal Aplasia (Hereditary), Blindness, or Degeneration, Congenital (see Retina, Amaurosis Congenita, Leger Type)
- Blindness or near blindness
Prenatal Exposure to Substances
State policy does not go beyond federal regulatory language.
Prematurity
Not included on state list of diagnosed/established conditions.
Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
List of diagnosed conditions includes:
- LBW </= 1200 g (qualifies children up to 2 years of age only)
- SGA (qualifies children up to 1 year of age only):
- GA 37-40 weeks and birth weight <2000 g (4 lbs, 6 oz)
- GA 36 weeks and birth weight <1875 g (4 lbs, 2 oz)
- GA 35 weeks and birth weight <1700 g (3 lbs, 12 oz)
- GA 34 weeks and birth weight <1500 g (3 lbs, 5 oz)
Prenatal Exposure to Zika
