Part C Developmental Delay Eligibility Criteria
Any delay for infants up to two months of age (adjusted for prematurity) or 20% (or one Standard Deviation below the mean) in one or more areas of development for infants and toddlers between two months and 36 months

- Auditory Neuropathy
- Bilateral or Unilateral hearing loss of > / = 25 dB at 2+ frequencies between 500-4000 Hz
- Mixed Hearing Loss
- Permanent Conductive Hearing Loss
- Sensorineural Hearing Loss
- Waardenburg Syndrome
Vision Impairment
Early On Established Conditions List includes:
- Amblyopia
- Cortical Visual Impairment (CVI)
- Low Vision (20/70)
- Nystagmus
- Retinopathy of Prematurity (ROP) (Stage 3-Stage5)
- Visual Field Loss
Prenatal Exposure to Substances
Early On Established Conditions List includes Exposures Affecting the Fetus/Child:
- Fetal alcohol exposure

Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
