Part C Developmental Delay Eligibility Criteria
1.5 SD below the mean, as measured by the Battelle Developmental Inventory - 2nd Ed. (BDI-2), in one or more developmental areas
At-Risk Served Under Federal Part C Policies
Children with 4 or more Infant and Toddler Risk Factors and/or Parental Risk Factors are eligible for 1 year under this criterion; factors include birth weight <1200 g* (less than 2 lbs 10.5 oz), GA <32 weeks* (developmental evaluation for eligibility will be based on CA, not on adjusted age), and in utero drug exposure (nicotine, ethanol, THC, cocaine, amphetamine, phenytoin, barbiturates, and other).
*These factors apply only to infants and toddlers who are under 18 months CA at the time of the evaluation for eligibility.

Hearing Impairment
Sensory conditions included in diagnosed conditions:
- Hearing loss - permanent
- Hearing impairment Bilateral, left ear, or right ear
- Neural hearing loss/auditory neuropathy
Vision Impairment
Sensory conditions included in diagnosed conditions:
- Blindness
- Both or one eye
- Cortical Blindness/Cortical Vision Impairment
- Optic Nerve Atrophy
- Retinopathy of Prematurity**
- Low Vision
- Low vision, both eyes 20/70 best acuity with correction
- Low vision, left eye, normal vision right eye**
- Low vision, right eye, normal vision left eye**
**These diagnoses or conditions may improve; they can be entered on only ONE evaluation and will have an eligibility timeframe of one year.
Prenatal Exposure to Substances
Early Intervention Diagnosed Conditions List includes Neonatal Abstinence Syndrome**, fetal alcohol effects, and Fetal Alcohol Syndrome.
**These diagnoses or conditions may improve; they can be entered on only ONE evaluation and will have an eligibility timeframe of one year.
Prematurity
Not included on state list of diagnosed/established conditions.
Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
Not included on state list of diagnosed/established conditions.
Prenatal Exposure to Zika
