1.5 Standard Deviations (SD) below the mean on the Battelle Developmental Inventory, 2nd Edition (BDI-2) in two developmental areas

Selected Categories of Diagnosed Conditions
Hearing Impairment
List of established medical conditions includes the following sensory impairments:
- Hearing impairment (25dB loss or greater) (389.--), unilateral or bilateral
- Auditory Neuropathy
- Central hearing loss (389.14)
Documentation of hearing loss must be by a doctor or audiologist.
Vision Impairment
List of established medical conditions includes the following sensory impairments:
- Profound impairment, both eyes (369.0-)
- Moderate or severe impairment, better eye, profound impairment lesser eye (369.1--)
- Moderate or severe impairment, both eyes (369.2)
- Legal blindness, as defined in USA (369.4)
- Retinopathy of prematurity (Grades 4 and 5) bilateral (362.21)
- Cortical Blindness (377.75), bilateral
Documentation of visual impairment must be by a doctor.
Prenatal Exposure to Substances
List of established medical conditions includes pre-natal exposures (Fetal Alcohol Syndrome, narcotics exposure, hallucinogenic agent exposure, cocaine exposure, and anticonvulsant exposure).
Prematurity
List of established medical conditions includes preterm infants 32 weeks or less Gestational Age up to 12 months of age at which point children preterm at birth must have a 1.5 standard deviation delay in one area of development for consideration of eligibility.
Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
List of established medical conditions includes VLBW (<1500 g at birth) up to 12 months of age at which point children preterm at birth must have a 1.5 standard deviation delay in one area of development for consideration of eligibility
