Part C Developmental Delay Eligibility Criteria
1.5 SD below the mean in two or more developmental areas or 2 SD below the mean in one or more developmental areas
At-Risk Served Under Federal Part C Policies
Single conditions make children eligible; list of at-risk conditions includes:
- LBW (1,200-1,500 g)
- NAS with evidence of clinical symptoms such as tremors, excessive high-pitched crying, hyperactive reflexes, seizures, and poor feeding
- SGA
Children at risk are only eligible for the services below (unless the child later becomes eligible as developmental delay or documented established condition):
- Abbreviated Individualized Family Support Plan (IFSP)
- Service coordination
- Developmental surveillance/monitoring (in partnership with community/medical home)
- Family support

- Auditory Neuropathy
- Aural Atresia (bilateral or unilateral)
- Sensorineural hearing loss in excess of 25 dB HL
Vision Impairment
Established Conditions list includes significant sensory impairment:
- Blindness ("legal" blindness or 20/200 best acuity with correction)
- Optic Nerve Hypoplasia (De Morsier's Syndrome): Septo Optic Dysplasia
- Retinopathy of Prematurity Stage III and/or IV (ROP) (Stage 3 unspecified [bilateral, left eye, right eye]; Stage 4 unspecified [bilateral, left eye, right eye]; Stage 5 unspecified [bilateral, left eye, right eye])
Prenatal Exposure to Substances
State policy does not go beyond federal regulatory language.
Prematurity
Not included on state list of diagnosed/established conditions.
Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
Established Conditions list includes LBW (<1,200 g at birth).
Prenatal Exposure to Zika
Established Conditions list includes Zika (confirmed congenital with symptoms; confirmed congenital no symptoms; probable congenital with symptoms; probable congenital no symptoms).
