Hearing Impairment
Established conditions include sensory Impairments:
- Hearing Impairment
- Neural hearing loss (includes auditory neuropathy).
Vision Impairment
Established conditions include sensory Impairments:
- Blindness ("legal" blindness or 20/200 best acuity with correction)
- Low Vision (20/70 best acuity with correction)
- Retinopathy of Prematurity, (Grades 4 and 5)
- Neurological Visual Impairment
Prenatal Exposure to Substances
Established conditions include pre-natal exposures (Fetal Alcohol Syndrome, Fetal Phenytoin (Dilantin) Syndrome).
Prematurity
Established conditions include prematurity (28 weeks or less gestation, up to 6 months CA only):
- <24 completed weeks of gestation
- 24 completed weeks of gestation
- 25-26 completed weeks of gestation
- 27-28 completed weeks of gestation.
Low Birth Weight, Very Low Birth Weight, Small for Gestational Age
Established conditions include VLBW (<1000 g at birth, up to 6 months CA only).
Prenatal Exposure to Zika
Not included on state list of diagnosed/established conditions.
