Step 7: The SLT finalizes a written plan

  1. Develop a written Implementation, scale-up and sustainability plan (Implementation Plan) with activities, timelines, roles, responsibilities and evaluation activities. Include in the plan key components of:
    • communication and dissemination,
    • training and technical assistance,
    • planned improvements in organizational structures, data and system supports
    • all evaluation activities; and
    • activities that address expansion (scale-up) and sustainability
  2. Identify and engage external expertise and additional resources as needed to support the implementation plan.
  3. Develop a progress monitoring and evaluation schedule that ensures the Implementation, Plan is regularly reviewed, updated and improved.
  4. Determined mechanisms for keeping stakeholders and the field apprised of progress.
